В соответствии с подпунктом 2) статьи 6 Кодекса Республики Казахстан от 7 июля 2020 года "О здоровье народа и системе здравоохранения" Правительство Республики Казахстан ПОСТАНОВЛЯЕТ:

1. Утвердить прилагаемый порядок, виды и объем медицинской помощи населению при чрезвычайных ситуациях, введении режима чрезвычайного положения.

2. Признать утратившими силу некоторые решения Правительства Республики Казахстан согласно приложению к настоящему постановлению.

3. Настоящее постановление вводится в действие по истечении десяти календарных дней после дня его первого официального опубликования.

Порядок, виды и объем медицинской помощи населению при чрезвычайных ситуациях, введении режима чрезвычайного положения

Глава 1. Общие положения

1. Порядок, виды и объем медицинской помощи населению при чрезвычайных ситуациях, введении режима чрезвычайного положения (далее – Порядок) разработаны в соответствии с Кодексом Республики Казахстан от 7 июля 2020 года "О здоровье народа и системе здравоохранения", законами Республики Казахстан от 8 февраля 2003 года "О чрезвычайном положении" и от 11 апреля 2014 года "О гражданской защите".

2. Настоящий Порядок определяет порядок предоставления, виды и объем медицинской помощи населению при чрезвычайных ситуациях, введении режима чрезвычайного положения.

3. В настоящем Порядке используются следующие основные понятия:

1) медицина катастроф – область медицины, направленная на предупреждение и ликвидацию медико-санитарных последствий чрезвычайных ситуаций социального, природного и техногенного характера, включающая предупреждение и лечение заболеваний населения, санитарно-противоэпидемические и санитарно-профилактические мероприятия, сохранение и восстановление здоровья участников ликвидации чрезвычайных ситуаций, а также медицинскую помощь сотрудникам аварийно-спасательных служб;

2) формирования медицины катастроф – предназначенные для работы в зонах чрезвычайных ситуаций, на территории, где введен режим чрезвычайного положения, отряды, группы, бригады и другие подразделения, создаваемые для оказания экстренной медицинской помощи и медицинской эвакуации;

3) экстренная медицинская помощь населению при чрезвычайных ситуациях – форма предоставления комплекса лечебно-профилактических мероприятий в сочетании с эвакуацией по назначению, осуществляемых при чрезвычайных ситуациях непосредственно в зоне чрезвычайной ситуации и/или на ее границах, на территории, где введен режим чрезвычайного положения;

4) медико-санитарные последствия чрезвычайных ситуаций – совокупность факторов и условий, характеризующих обстановку, сложившуюся при чрезвычайных ситуациях, введении режима чрезвычайного положения, и определяющих содержание, объем и организацию медицинской помощи населению. Основными ее элементами являются: величина и структура санитарных потерь населения, закономерности их формирования; нуждаемость пораженных (больных) в медицинской помощи, санитарно-гигиеническое и санитарно-эпидемическое состояние зоны (района) чрезвычайной ситуации, территории, где введен режим чрезвычайного положения, потребность в силах и средствах для медико-санитарного обеспечения и их наличие; состояние медицинских сил и средств; условия работы медицинских и аварийно-спасательных формирований при ликвидации чрезвычайной ситуации, условия жизнеобеспечения населения и другие;

5) вид медицинской помощи при чрезвычайных ситуациях – перечень лечебно-профилактических мероприятий, определяемый местом их проведения, квалификацией лиц, оказывающих медицинскую помощь, и наличием медицинского оснащения;

6) объем экстренной медицинской помощи – полная или сокращенная по жизненным показаниям совокупность лечебно-профилактических мероприятий, выполняемых в рамках вида медицинской помощи в зависимости от условий обстановки в зоне чрезвычайной ситуации, на территории, где введен режим чрезвычайного положения, и возможностей этапа медицинской эвакуации.

Глава 2. Порядок предоставления медицинской помощи при чрезвычайных ситуациях, введении чрезвычайного положения

4. Медицинская помощь населению при чрезвычайных ситуациях, введении режима чрезвычайного положения предоставляется в зависимости от классификации чрезвычайной ситуации, а также характера и масштабов их медико-санитарных последствий.

5. При объектовых чрезвычайных ситуациях оказание медицинской помощи пораженным (больным) осуществляется организациями здравоохранения в режиме повседневной деятельности.

6. При возникновении местных, региональных и глобальных чрезвычайных ситуаций, а также введении режима чрезвычайного положения вводятся в действие силы и средства медицинской службы гражданской защиты.

7. Медицинская помощь при чрезвычайных ситуациях, введении режима чрезвычайного положения предоставляется методом этапного лечения пораженных (больных) в соответствии с характером поражения, степенью тяжести в сочетании с медицинской эвакуацией по назначению.

8. На первом этапе непосредственно в зоне чрезвычайной ситуации и/или на ее границах, на территории, где введен режим чрезвычайного положения, экстренная медицинская помощь оказывается силами службы экстренной медицинской и психологической помощи уполномоченного органа в сфере гражданской защиты, аварийно-спасательных подразделений, формирований медицины катастроф и медицинской службы гражданской защиты, а также территориальных медицинских организаций, сохранивших свою работоспособность.

9. На втором этапе медицинская помощь оказывается в организациях здравоохранения, находящихся за пределами зоны чрезвычайной ситуации, территории, где введен режим чрезвычайного положения.

10. Для оказания медицинской помощи при чрезвычайных ситуациях, введении режима чрезвычайного положения могут быть привлечены и использованы персонал и материально-технические средства организаций здравоохранения, юридических и физических лиц, занимающихся медицинской и фармацевтической деятельностью, а также транспортных организаций в порядке, установленном законодательством Республики Казахстан, и в соответствии с настоящим Порядком.

11. Для проведения мероприятий экстренной медицинской помощи в организациях здравоохранения заблаговременно создаются и содержатся медицинские формирования гражданской защиты. Порядок создания медицинских формирований определяется уполномоченным органом в сфере гражданской защиты по согласованию с уполномоченным органом в области здравоохранения.

12. Прием пораженных (больных) из зоны чрезвычайной ситуации, территории, где введен режим чрезвычайного положения, дальнейшее оказание медицинской помощи, лечение и медицинская реабилитация осуществляются организациями здравоохранения в соответствии с законодательством Республики Казахстан.

13. Координация и взаимодействие государственных органов и организаций здравоохранения в сфере медицины катастроф осуществляются уполномоченным органом в сфере гражданской защиты.

Глава 3. Виды и объем медицинской помощи при чрезвычайных ситуациях, введении чрезвычайного положения

14. Медицинская помощь пораженным (больным) при чрезвычайных ситуациях, введении режима чрезвычайного положения в зависимости от места ее оказания, квалификации лиц, ее оказывающих, и обеспеченности медицинским имуществом предоставляется в виде доврачебной, скорой и специализированной медицинской помощи.

15. Объем медицинской помощи при чрезвычайных ситуациях, введении режима чрезвычайного положения в зависимости от складывающейся медико-санитарной и кризисной обстановки в зоне чрезвычайной ситуации, на территории, где введен режим чрезвычайного положения, в рамках установленных видов медицинской помощи определяется по медицинским показаниям на основе профилактических, диагностических и лечебных мероприятий, обладающих наибольшей доказанной эффективностью.

16. Непосредственно в зоне чрезвычайной ситуации и/или на ее границах, на территории, где введен режим чрезвычайного положения, оказывается следующая медицинская помощь:

1) доврачебная – медицинская помощь, оказываемая средними медицинскими работниками самостоятельно или в составе мультидисциплинарной команды, включающая в себя оценку состояния пациента, постановку доврачебного диагноза, исполнение доврачебных манипуляций и процедур и уход за пострадавшими;

2) скорая – медицинская помощь, оказываемая фельдшерскими и специализированными (врачебными) бригадами, включающая в себя оценку состояния пациента, постановку диагноза, оказание медицинской помощи и транспортировку в медицинские организации;

3) специализированная – медицинская помощь, оказываемая профильными специалистами при заболеваниях, требующих специальных методов диагностики, лечения, медицинской реабилитации.

Приложение к постановлению Правительства Республики Казахстан от 26 ноября 2020 года № 796

Перечень утративших силу некоторых решений Правительства Республики Казахстан

1. Постановление Правительства Республики Казахстан от 17 июня 2010 года № 608 "Об утверждении Правил предоставления, видов и объема медицинской помощи при чрезвычайных ситуациях, введении режима чрезвычайного положения" (САПП Республики Казахстан, 2010 г., № 39, ст. 341).

2. Постановление Правительства Республики Казахстан от 14 ноября 2013 года № 1226 "О внесении изменений в постановление Правительства Республики Казахстан от 17 июня 2010 года № 608 "Об утверждении Правил предоставления, видов и объема медицинской помощи при чрезвычайных ситуациях" (САПП Республики Казахстан, 2013 г., № 65, ст. 890).

3. Пункт 10 изменений и дополнений, которые вносятся в некоторые решения Правительства Республики Казахстан, утвержденных постановлением Правительства Республики Казахстан от 2 апреля 2015 года № 173 "О внесении изменений и дополнений и признании утратившими силу некоторых решений Правительства Республики Казахстан и распоряжения Премьер-Министра Республики Казахстан от 7 апреля 2011 года № 44-р "О порядке организации и проведения спасательных и других неотложных работ в зонах возможных землетрясений" (САПП Республики Казахстан, 2015 г., № 18-19, ст. 94).

In accordance with subclause 2) of article 6 of the Code of the Republic of Kazakhstan dated July 7, 2020 "On Public Health and Healthcare System" the Government of the Republic of Kazakhstan HEREBY RESOLVES:

1. To approve the attached procedure, types and amount of medical assistance to the population in emergency situations, the introduction of a state of emergency.

2. To recognize as invalid certain decisions of the Government of the Republic of Kazakhstan according to the appendix to this Resolution.

3. This Resolution shall come into force upon expiration of ten calendar days after the date of its first official publication.

Chapter 1. General provisions

1. The procedure, types and amount of medical assistance to the population in emergency situations, the introduction of a state of emergency (hereinafter referred to as the Procedure) have been developed in accordance with the Code of the Republic of Kazakhstan dated July 7, 2020 "On Public Health and Healthcare System", laws of the Republic of Kazakhstan dated February 8, 2003 "On State of Emergency" and dated April 11, 2014 "On Civil Protection".

2. This Procedure shall determine the procedure for provision, types and amount of medical assistance to the population in emergency situations, the introduction of a state of emergency.

3. In this Procedure, the following basic concepts are used:

1) disaster medicine - the field of medicine aimed at preventing and eliminating the medical and sanitary consequences of emergencies of a social, natural and man-made nature (hereinafter - emergencies), including the prevention and treatment of diseases of the population, sanitary-anti-epidemic and sanitary-preventive measures, preservation and restoration of health of participants in the liquidation of emergency situations, as well as medical assistance to employees of emergency services;

2) disaster medicine units – designed to work in emergency zones, in the territory where the state of emergency has been introduced, detachments, groups, brigades and other units created to provide emergency medical care and medical evacuation;

3) emergency medical assistance to the population in emergency situations - a form of providing a complex of therapeutic and prophylactic measures in combination with evacuation by appointment, carried out in emergency situations directly in the emergency zone and/or on its borders, in the territory where the state of emergency is introduced;

4) health and sanitary consequences of emergency situations – a set of factors and conditions that characterize the situation in emergency situations, the introduction of a state of emergency, and determine the content, volume and organization of medical assistance to the population. Its main elements are: the size and structure of sanitary losses of the population, the common factors of their formation; the need of the affected (patients) for medical care, the sanitary-hygienic and sanitary-epidemic state of the emergency zone (area), the territory where the state of emergency is introduced, the need for forces and means for health and sanitary provision and their availability; the state of medical forces and means; working conditions of medical and emergency rescue teams during emergency response, living conditions of the population and other;

5) type of medical assistance in emergency situations – a list of medical and preventive activities, determined by the place of their implementation, the qualifications of the persons providing medical assistance, and the availability of medical equipment;

6) volume of emergency medical assistance – a complete or reduced for life indications set of therapeutic and prophylactic measures carried out within the framework of the type of medical assistance, depending on the conditions of the situation in the emergency zone, in the territory where the state of emergency is introduced, and the possibilities of the medical evacuation stage.

Chapter 2.
Procedure for provision of medical assistance in emergency situations, the introduction of a state of emergency

4. The medical assistance to the population in emergency situations, the introduction of a state of emergency shall be provided depending on the classification of an emergency situation, as well as the nature and the scope of their health and sanitary consequences.

5. In case of facility-based emergency situations, the provision of medical assistance to the affected (sick) shall be carried out by health organizations in a daily activity mode.

6. In the event of local, regional and global emergency situations, as well as the introduction of a state of emergency, the forces and means of the medical civil protection service shall activate.

7. Medical assistance in emergency situations, the introduction of a state of emergency shall be provided by the method of staged treatment of the affected (patients) in accordance with the nature of the damage, the severity in combination with medical evacuation as necessary.

8. At the first stage, directly in the emergency zone and/or at its borders, in the territory where the state of emergency is introduced, emergency medical assistance shall be provided by the emergency medical and psychological assistance service of the authorized body in the field of civil protection, emergency rescue units, disaster medicine units and the medical service of civil protection, as well as territorial medical organizations that have retained their working capacity

9. At the second stage, medical assistance shall be provided in healthcare organizations located outside the emergency zone, the territory where the state of emergency is introduced.

10. For the provision of medical assistance in emergency situations, the introduction of a state of emergency, personnel and material and technical means of healthcare organizations, legal entities and individuals engaged in medical and pharmaceutical activities, as well as transport organizations may be involved and used in accordance with the procedure, prescribed by the legislation of the Republic of Kazakhstan, and in accordance with this Procedure.

11. To carry out emergency medical care in healthcare organizations, civil protection medical units shall be created and maintained in advance. The procedure for creating medical units shall be determined by the authorized body in the field of civil protection in agreement with the authorized body in the field of healthcare.

12. Reception of the injured (the sick) from the emergency zone, the territory where the state of emergency is introduced, further medical assistance, treatment and medical rehabilitation shall be carried out by health organizations in accordance with the legislation of the Republic of Kazakhstan.

13. Coordination and interaction between the state bodies and healthcare organizations in the field of disaster medicine shall be carried out by the authorized body in the field of civil protection.

Chapter 3.
Types and the volume of medical assistance in emergency situations, the introduction of a state of emergency

14. Medical assistance to the injured (the sick) in emergency situations, the introduction of a state of emergency depending on the place of its provision, qualification of persons, providing it, and provision with medical property, shall be provided in the form of first-aid, emergency and specialized medical care.

15. The volume of medical assistance in emergency situations, the introduction of a state of emergency, depending on the emerging medical and sanitary and crisis situation in the emergency zone, in the territory where the state of emergency is introduced, within the established types of medical care, shall be determined according to medical indications on the basis of preventive, diagnostic and therapeutic measures with the greatest proven efficacy.

16. Immediately in the emergency zone and/or on its borders, in the territory where the state of emergency is introduced, the following medical assistance shall be provided:

1) Paramedical service, provided by paramedical workers independently or as part of a multidisciplinary team, including an assessment of the patient's condition, making a pre-medical diagnosis, performing pre-medical manipulations and procedures, and caring for the injured;

2) Emergency health service provided by paramedic and specialized (medical) teams, including an assessment of the patient's condition, diagnosis, medical care and transportation to medical organizations;

3) Specialized - medical care provided by specialized specialists for diseases requiring special methods of diagnosis, treatment, medical rehabilitation.

1. Resolution of the Government of the Republic of Kazakhstan dated June 17, 2010 No. 608 "On approval of the Rules for the provision, types and volume of medical assistance in emergency situations, the introduction of a state of emergency" (Collected Acts of the President and the Government of the Republic of Kazakhstan, 2010, No. 39, art. 341).

2. Resolution of the Government of the Republic of Kazakhstan dated November 14, 2013 No. 1226 "On amendments to the resolution of the Government of the Republic of Kazakhstan dated June 17, 2010 No. 608 " On approval of the Rules for the provision, types and volume of medical assistance in emergency situations, the introduction of a state of emergency" (Collected Acts of the President and the Government of the Republic of Kazakhstan, 2013, No.65, art. 890).
