"Халық денсаулығы және денсаулық сақтау жүйесі туралы" Қазақстан Республикасының Кодексі 75-бабының 3-тармағына, "Мемлекеттік статистика туралы" Қазақстан Республикасының Заңы 4-бабының 2) тармақшасына, 13-бабына және 16-бабы 3-тармағының 2) тармақшасына сәйкес БҰЙЫРАМЫН:

1. Осы бұйрыққа қосымшаға сәйкес, денсаулық сақтау саласындағы статистикалық байқауды жүзеге асыру қағидаларын, денсаулық сақтау саласындағы статистикалық есепке алу мен есептілік нысандарын, оларды жүргізу, толтыру тәртібін және ұсыну мерзімдерін бекіту туралы қағидалары бекітілсін.

2.Қазақстан Республикасы Денсаулық сақтау министрлігінің Медициналық көмекті ұйымдастыру департаментіне Қазақстан Республикасының заңнамасында белгіленген тәртіппен:

1) осы бұйрықты Қазақстан Республикасының Әділет министрлігінде мемлекеттік тіркеуді;

2) осы бұйрықты ресми жарияланғаннан кейін оны Қазақстан Республикасы Денсаулық сақтау министрлігінің интернет-ресурсында орналастыруды;

3) осы бұйрықты мемлекеттік тіркегеннен кейін он жұмыс күні ішінде Қазақстан Республикасы Денсаулық сақтау министрлігінің Заң департаментіне осы тармақтың 1) және 2) тармақшаларында көзделген іс-шаралардың орындалу туралы мәліметтерді ұсынуды қамтамасыз етсін.

3. Осы бұйрықтың орындалуын бақылау жетекшілік ететін Қазақстан Республикасының Денсаулық сақтау вице-министріне жүктелсін.

4. Осы бұйрық алғашқы ресми жарияланған күнінен бастап күнтізбелік он күн өткен соң қолданысқа енгізіледі.

1-тарау. Жалпы ережелер

1. Осы денсаулық сақтау саласындағы статистикалық байқауды жүзеге асыру қағидалары, денсаулық сақтау саласындағы статистикалық есепке алу мен есептілік нысандарын, оларды жүргізу, толтыру тәртібі және ұсыну мерзімдері (бұдан әрі – Қағидалар) "Халық денсаулығы және денсаулық сақтау жүйесі туралы" Қазақстан Республикасы Кодексінің (бұдан әрі – Кодекс) 75-бабының 3-тармағына, "Мемлекеттік статистика туралы" Қазақстан Республикасының Заңы (бұдан әрі – Заң) 4-бабының 2) тармақшасына, 13-бабына және 16-бабы 3-тармағының 2) тармақшасына сәйкес әзірленді және денсаулық сақтау саласындағы статистикалық байқауды жүргізу тәртібін, денсаулық сақтау саласындағы статистикалық есепке алу мен есептілік нысандарын, оларды жүргізу тәртібін, толтыру мен ұсыну мерзімдерін айқындайды.

2-тарау. Денсаулық сақтау саласындағы статистикалық байқауды жүзеге асыру тәртібі, денсаулық сақтау саласындағы статистикалық есепке алу мен есептілік нысандары, оларды жүргізу, толтыру тәртібі және ұсыну мерзімдері

2. Денсаулық сақтау саласындағы статистикалық байқау халықтың денсаулығы көрсеткіштерінің және денсаулық сақтау ұйымдары қызметінің динамикасын зерделеу мақсатында жүргізіледі. Жинаудың және өңдеудің жаңа технологияларын қолдана отырып, есепке алудың, есептіліктің, медициналық-статистикалық ақпараттың бірыңғай жүйесін қалыптастыруды, сондай-ақ денсаулық сақтау саласындағы уәкілетті органның статистикалық деректерді ұсынуын "Цифрлық денсаулық сақтау субъектілерінің дербес медициналық деректерді жинауды, өңдеуді, сақтауды, қорғауды және беруді жүзеге асыру қағидаларын бекіту туралы" Қазақстан Республикасы Денсаулық сақтау министрінің 2021 жылғы 14 сәуірдегі № ҚР ДСМ-30 бұйрығының (Нормативтік құқықтық актілерді мемлекеттік тіркеу тізілімінде № 32015 болып тіркелген) 28-тармағына сәйкес дербес медициналық деректер агрегаторы жүзеге асырады.

3. Денсаулық сақтау саласындағы статистикалық есепке алу және есептілік нысандары, оларды жүргізу, толтыру тәртібі және оларды ұсыну мерзімдері "Денсаулық сақтау саласындағы есепке алу құжаттамасының нысандарын, сондай-ақ оларды толтыру жөніндегі нұсқаулықтарды бекіту туралы" Қазақстан Республикасы Денсаулық сақтау министрінің міндетін атқарушының 2020 жылғы 30 қазандағы № ҚР ДСМ-175/2020 (Нормативтік құқықтық актілерді мемлекеттік тіркеу тізілімінде № 21579 болып тіркелген), "Денсаулық сақтау саласындағы есептік құжаттама нысандарын бекіту туралы" Қазақстан Республикасы Денсаулық сақтау министрінің 2020 жылғы 22 желтоқсандағы № ҚР ДСМ-313/2020 (Нормативтік құқықтық актілерді мемлекеттік тіркеу тізілімінде № 21879 болып тіркелген) бұйрықтарына сәйкес айқындалады.

4. Денсаулық сақтау саласындағы статистикалық байқау "Денсаулық сақтау саласындағы көрсеткіштерді қалыптастыру (есептеу) әдістемесін бекіту туралы" Қазақстан Республикасы Денсаулық сақтау министрінің 2020 жылғы 30 қарашадағы № ҚР ДСМ-212/2020 бұйрығына (Нормативтік құқықтық актілерді мемлекеттік тіркеу тізілімінде № 21698 болып тіркелген) сәйкес жүргізіледі.

5. Денсаулық сақтау субъектілерінің әкімшілік деректерін жинауға арналған нысандар дербес медициналық деректер агрегаторының аумақтық филиалдарында аудандық, облыстық деңгейде жиынтық нысандарда қалыптастырылады.

6. Дербес медициналық деректер агрегаторының аумақтық филиалдары денсаулық сақтау субъектілерінің әкімшілік деректерін жинауға арналған нысандарды дербес медициналық деректер агрегаторына ұсынады.

7. Дербес медициналық деректер агрегаторы денсаулық сақтау субъектілерінің әкімшілік деректерін жинауға арналған нысандарды қабылдайды және кейіннен осы қағидалардағы № 1, 2, 3, 4 қосымшаға сәйкес денсаулық сақтау саласындағы уәкілетті органға ұсына отырып, Қазақстан Республикасы бойынша жиынтық нысандарды қалыптастырады.

8. Осы қағидалардағы № 1, 2, 3, 4 қосымшаларға сәйкес өңірлер бөлінісінде денсаулық сақтау саласындағы уәкілетті орган денсаулық сақтау субъектілерінің әкімшілік деректерін жинауға арналған нысандарды және Қазақстан Республикасы бойынша жиынтық нысандарды мемлекеттік статистика жөніндегі уәкілетті органға ұсынады.

9. Осы қағидалардағы № 1, 2, 3, 4 қосымшаларға сәйкес өңірлер бөлінісінде денсаулық сақтау саласындағы уәкілетті орган денсаулық сақтау субъектілерінің әкімшілік деректерін жинауға арналған нысандардың статистикалық деректерін және Қазақстан Республикасы бойынша жиынтық нысандарды өңірлер бөлінісінде және Қазақстан Республикасы бойынша халықтың денсаулығы мен денсаулық сақтау ұйымдары қызметінің көрсеткіштерін есептеу, статистикалық байқау жүргізу үшін пайдаланады.

In accordance with paragraph 3 of Article 75 of the Code of the Republic of Kazakhstan "On public health and health care system", subparagraph 2) of Article 4, Article 13 and subparagraph 2) of paragraph 3 of Article 16 of the Law of the Republic of Kazakhstan “On State Statistics”, I HEREBY ORDER:

1. To approve the rules for statistical observation in the field of healthcare, the forms of statistical recording and reporting in the field of healthcare, the procedure for their maintenance, filling out and deadlines for their submission in accordance with the appendix to this order.

2. In the manner prescribed by the legislation of the Republic of Kazakhstan, the Medical Aid Department of the Ministry of Healthcare of the Republic of Kazakhstan shall ensure:

1) the state registration of this order with the Ministry of Justice of the Republic of Kazakhstan;

2) the posting of this order on the website of the Ministry of Healthcare of the Republic of Kazakhstan;

3) the submission of information on the implementation of the measures provided for in subparagraphs 1) and 2) of this paragraph to the Legal Department of the Ministry of Healthcare of the Republic of Kazakhstan within ten working days of the state registration of this order with the Ministry of Justice of the Republic of Kazakhstan.

3. Control over the execution of this order shall be entrusted to the supervising deputy minister of healthcare of the Republic of Kazakhstan.

4. This order comes into effect ten calendar days of its first official publication.

Chapter 1. General provisions

1. These rules for statistical observation in the field of health care, forms of statistical recording and reporting in the field of health care, the procedure for their maintenance, filing out and deadlines for submission (hereinafter - Rules) are developed in accordance with paragraph 3 of Article 75 of the Code of the Republic of Kazakhstan "On the Health of the People and the Health Care System" (hereinafter - the Code), subparagraph 2) of Article 4, Article 13 and subparagraph 2) of paragraph 3 of Article 16 of the Law of the Republic of Kazakhstan "On State Statistics" (hereinafter - the Law) and determine the procedure for statistical observation in the field of health care, forms of statistical recording and reporting in the field of health care, the procedure for their maintenance, filling out and deadlines for their submission (hereinafter - the Rules).

Chapter 2. Procedure for statistical observation in the field of health care, forms of statistical recording and reporting in the field of health care, the procedure for their maintenance, filling out and deadlines for their submission

2. Statistical observation in the field of health care is conducted in order to study the dynamics of health indicators of the population and activities of health care organizations. Formation of a unified system of accounting, reporting, medical and statistical information using new technologies of collection and processing, as well as the provision of statistical data by the authorized body in the field of health care, shall be carried out by the aggregator of personal medical data in accordance with paragraph 28 of the order of the Minister of Health of the Republic of Kazakhstan from April 14, 2021 № KR DSM-30 "On approval of the rules of collection, processing, storage, protection and provision of personal medical data by the subjects of digital health care" (registered with the Register of State registration of regulatory legal acts under № 32015).

3. Forms of statistical observation in the field of health care, forms of statistical recording and reporting in the field of health care, the procedure for their maintenance, filling out and deadlines for their submission shall be determined in accordance with orders of the Acting Minister of Healthcare of the Republic of Kazakhstan dated October 30, 2020 № ҚР ДСМ-175/2020 "On the approval of forms of accounting documentation in the field of health care, as well as instructions for their completion" (registered with the Register of State registration of regulatory legal acts under № 21579), of the Minister of Healthcare of the Republic of Kazakhstan dated December 22, 2020 № ҚР ДСМ-313/2020 "On approval of the forms of reporting documentation in the field of health care" (registered with the Register of State registration of regulatory legal acts under № 21879).

4. Statistical observation in the field of health care shall be conducted in accordance with the order of the Minister of Healthcare of the Republic of Kazakhstan dated November 30, 2020 № ҚР ДСМ-212/2020 "On approval of the methodology of formation (calculation) of indicators in the field of health care" (registered with the Register of State registration of regulatory legal acts under № 21698).

5. Forms intended for the collection of administrative data of health subjects are formed into consolidated forms at the district, regional level in the territorial branches of the aggregator of personal health data.

6. Territorial branches of the aggregator of personal health data provide forms designed to collect administrative data of health subjects to the aggregator of personal health data.

7. Aggregator of personal health data accepts forms intended for collection of administrative data of health subjects and forms consolidated forms for the Republic of Kazakhstan with subsequent submission to the authorized body in the field of health according to Annexes 1, 2, 3, 4 to these Rules.

8. The authorized body in the field of health care provides the forms intended for collection of administrative data of the subjects of health care, by regions and summary forms on the Republic of Kazakhstan to the authorized body on state statistics according to Annexes 1, 2, 3, 4 to these Rules.

9. Statistical data of the forms intended for collection of administrative data of the subjects of health care, in the context of regions and consolidated forms on the Republic of Kazakhstan the authorized body in the field of health care uses for carrying out calculations of indicators of population health and activity of health care organizations, statistical observation in the context of regions and on the Republic of Kazakhstan according to Annexes 1, 2, 3, 4 to these Rules.
